In the event you are unable to reach me, in the case of accident or injury, I give my permission for treatment as deemed necessary by staff or emergency personnel.  I also release Georgia Dance Force/Healthy Youth USA and its staff of liability in case of injury or accident incurred

I give permission for my child image, video or voice to be used for any photographs, videos and/or any publications that Georgia Dance Force (and any affiliates) may seek to copyright, use, broadcast or publish.
